You or your family member may be eligible for CHSP!
The Commonwealth Home Support Programme (CHSP) is Australia's national funding program to help older people stay independent and in their homes and communities. CHSP works with you to maintain your independence rather than doing things for you.
Avanti is currently delivering CHSP-funded programs on the Gold and Sunshine Coasts and hopes to expand to other locations in the near future.
To receive CHSP funding, you need to:
1. register with My Aged Care
2. have a Regional Assessment Services (RAS) assessment.
Call My Aged Care on 1800 200 422. Have your Medicare card ready and allow at least 10 minutes to go through the questions they will ask.
If you are a carer or family member helping someone else with My Aged Care, you will need their consent to speak on their behalf. See My Aged Care Portal - Talking to My Aged Care for someone.
My Aged Care will refer you for a home support assessment with a Regional Assessment Service (RAS) if you need low-level support to stay independent in your home.
Once My Aged Care refers you for an assessment, an assessor will contact you to arrange a home visit. This usually occurs within 2-3 weeks of your referral.

An assessor will visit you at your own home to look at:
daily tasks – they’ll ask you to show them how you are doing them now, and where you are finding things difficult
goals and aspirations - they want to know what's important to you
health and well-being - they can help with any health concerns
carers and community - to understand your support system and needs.
After your assessment, a home support plan is developed, which is tailored to your individual needs, with the aim of helping you live as independently as possible. The RAS can include the relevant code(s) for allied health and refer directly to Avanti via the My Aged Care portal.
There are 2 broad categories for Avanti’s exercise-based services under this funding:
1. Wellness – this is for people who need to build/maintain strength, balance, mobility, social connection, etc, and can take the form of regular ongoing 1-on-1 sessions with our exercise physiologists (EPs);
2. Reablement – this is for people recovering from injury/illness/hospitalisation. We will offer 3-month reablement packages which incorporate 1-on-1 sessions with our EPs plus unlimited access to our gyms during that time.
The good news is that this is additional funding over and above any funds you may receive for transport, domestic assistance, etc, so it’s a great way to invest in your own wellness/reablement.
