What a home visit covers

Support in the home, assessed in person

A home visit looks at how the person manages in their actual home and community — daily living, routines, transfers and safety — rather than how they perform in a clinic room. Considering support at home on the Sunshine Coast? Tell us who needs support and what has become difficult, and the team explains whether a home visit with Kayla or Lin is the right next step.

Functional assessments and reports

In-home and community assessment of what a person does day to day, with reports that connect recommendations to everyday life — including NDIS Functional Capacity Assessments.

Equipment and assistive technology

Advice and prescription where equipment may help with safety, independence or participation. Funding, trial and supply responsibilities are clarified before anything is ordered.

Home modifications and daily living support

Whether changes to the home may help, plus ongoing therapy for daily living and participation goals. We do not build or install home modifications. Recommendations follow the OT assessment.

Describe what is difficult at home and the Sunshine Coast team talks it through with you — explaining whether a home visit is the right next step before recommendations are discussed.
